كما ذكرنا فى موضوع سابق اننا سنعرض اثر مرض السكر على الانسان
واليوم سنتكلم على اثره على العين وسلبياته
اذا عرفت أنك مريض بالسكر فيجب ان تضع يدك على عينك وتذهب الى طبيب العيون
فاذا قال لك الطبيب عينك سليمة وان الشبكية خالية من أي اصابة فليس معنى ذلك الا تكشف على عينك
فمن الضروري الكشف على العين بعد ذلك بانتظام
فالشبكة التي كانت أمس قد تكون قد اصبيت اليوم
وهكذا من الضروري دائما ان نتأكد من سلامة عينك اذا كنت مريضا بالسكر
وتغيرات الشبكية الناجمة من السكر اصبحت ظاهرة هامة لهذا السبب ذلك انها متعلقة بمدة مرض السكر نفسه وتصيب المرضى بصرف النظر عن ضبط علاج السكر وان كان المعقول أنه يصيب أكثر المرضى غير منتظمي العلاج ومن هم مصابون ايضا بأمراض الأوعية من تصلب وارتفاع ضغط الدم
وطبيعة المرض أي تغيرات الشبكية غير معروفة تماما وهي تظهر في ادق الاوعية الدموية بالشبكية فتصبح ضعيفة الجدار وعرضة لأن تؤدي الى نتيجيتين هما الرشح والنزف خاصة اذا تعددت الأوعية وظهرت بها حويصلات دموية وثبت منها العديد على سطح الشبكية
ومن الناحية الاكلينيكية تنقسم تغيرات الشبكية الى نوعين هامين وهو ما اتفق عليه وهما نوع حميد بطيء التقدم ونوع خبيث سريع التقدم يؤدي الى تطورات سريعة وضعف شديد في النظر وقد ينتهي بالانفصال الشبكى التليفى
ما هي نسبة الاصابة بالمضاعفات الشكبية ؟
تصل هذه النسبة فى بلدنا كغيرها من بلاد العالم من 10-15 بالمائة من مرضى السكر لكل الاعمار وكل أزمنة المرض وحدوث المضاعفات كما قلنا متعلق بطول هذه الاصابة بمرض السكر فان عاش المريض العمر العادي كما يحدث الآن فان نسبة الاصابة ترتفع
ولكن هل كل هؤلاء عرضة لاخطار هذه التغيرات على النظر

والتغيرات الخبيثة هذه تبدأ في أوعية دموية مريضة على الشبكية رقيقة الجدار سريعة النزف والرشح وتتكاثر ويصحبها تليف وبذلك تتوالى التغيرات بخطوات سريعة وتهدد البصر من كثرة التليف والتعرض الشديد للنزف بالجسم الزجاجي ما يتبعه من حجب الرؤية ومزيد من التليف وأخيرا الانفصال الشبكي
وما هي الوقاية اذن وما هو العلاج؟
من ناحية الوقاية فان النصيحة العامة هي علاج السكر بقدر المستطاع وكذلك ضغط الدم المرتفع
ولكن الأهم هو فحص قاع العين عند اكتشاف مرض السكر وبعد ذلك دوريا لاكتشاف التغييرات الشبكية ونوعها وتقرير ما يلزم لها
أما العلاج فهو الآن احسن كثيرا مما سبق في العقد الماضي اذ تبين ان الكي الضوئي وسيلة مفيدة في كثير من الحالات ان أجري بالقدر اللازم وفي الوقت في المناسب اذ به يمكن التخلص من كثير من التغيرات الضارة بالشبكية والتي هي مصدر الرشح والنزف

واما الكي الضوئي فانه ايضا مفيد قطعا في الكثير من الحالات ولا يشكل خطرا على العين بشرط حسن استعماله من حيث احتيار المريض المناسب في الوقت المناسب
والكي الكهربائي من الوسائل التي تؤدي غرض الكي الضوئي ان تعذر اجراؤه ولذا يمكن اجراؤه في بعض الحالات الا ان هذا الأخير يحتاج الى جراحة بسيطة بالمقارنة بالكي الضوئي الذي يجري بدون الجراحة
